COVID-19 Vaccine Survey Tool

With the recent rollout of the Moderna and Pfizer COVID-19 vaccines, many are wondering: “when will I be able to get vaccinated?”

While we don’t know the exact answer to that just yet, we’ve developed a new vaccine survey tool to inform residents of their turn as soon as possible and get people set up with their appointments.

JACOHD is following the state of Missouri’s COVID-19 vaccine distribution plan. The time it takes to make a COVID-19 vaccine available to everyone who wants one is dependent on a few factors.

  1. The supply of vaccine provided by the state of Missouri to administrators in our area. 
  2. Where residents fall within the phased distribution plan.
    • The first phase focuses on (1A) long-term care facility residents and staff, patient-facing healthcare workers, and (1B) high-risk individuals, first responders, and essential workers. 
    • The state will not move to the next phase until the vaccine is made available to all individuals in the previous phase who want one.

The survey asks for contact information, occupation, and pre-existing medical conditions. Responses from the survey will help us determine which phase of the state’s vaccination plan residents fall under. All individuals who live or work in Jackson County and are interested in receiving the vaccine are encouraged to fill out the survey here.

As the vaccine becomes available to each tier, we will contact individuals who completed the survey with further instructions on scheduling an appointment. 

Per the guidance provided by the CDC, COVID-19 vaccinations will not be provided to those under the age 16.

Become a Vaccine Provider

Are you a healthcare provider? Are you interested in being an administrator of a COVID-19 vaccine? 

As a healthcare provider, you may be eligible to register through the Missouri Department of Health and Senior Services (MO DHSS) as a COVID-19 vaccine provider. Whether you’re vaccinating a few staff members or the larger community, no organization is too small to become a vaccine provider.
